FOR THE YEAR 1874-75.                                     13

were 35 of them, and when they returned at the end of the season most of them expressed
a desire to proceed next year further west into the Sirgoojah and Juspore states. The
operations of apprentices were distributed with the parties, and amounted to 10,605 cases and
9,761 successful, or 94.3 per cent.

Sum total.

" 20. Taken as a whole, the agency as paid by the state has afforded a total throughout
the circle of 59,818 cases, of which 51,518 may be taken as successful. The average
ascertained successful cases among the examined was 94.31 per cent., and the percentage has
been used for the whole. If we place against this number the cost of inspection, the cost per
case comes to six pie.

General
conclusion.

" There is no doubt that with a little judicious cultivation the unpaid agency could be
made to produce a very much larger return, but it will require great care and not a little labour
and perseverance. Meanwhile the tendency of the Manbhoom men to pass over towards the
tributary states to the west, which are without the circle, should not be lost sight of, but their
annual immigration regularly encouraged as long as there is no supply of resident vaccinators
to take their place.

Superintendence
necessary

" 21. Native superintendence for the unpaid agency.—Three or four native superintendents
to each district would be found of great use, and, with the aid of these, the civil surgeons or
superintendent would be in a better position to judge of the state of vaccination in the
interior of districts, and could at least more easily provide for any outbreak of small-pox
which might occur. Taking the cases of vaccination which has been done this season in

* 94.31 per cent.

Chota Nagpore by all agencies, excepting the dispensaries, the reports from all of which I have
not seen, and putting a fair estimate* on those the success of
which is not accurately known, we have 87,161 cases with
76,258 successful, and this represents 22.3 per 1,000 of the population, or seven-tenths of the
birth-rate (reckoned at 30 per 1,000) of the four districts."

Total results for
the division

46. These reports of the civil surgeons, of which abstracts are furnished below, afford
some additional information. Including dispensary vaccination, it appears that 61,287
operations with a very fair percentage of success, were performed by agencies other than the
circle establishments (70,998 successful), bringing up the total of operations to 88,630.
Under proper supervision and judicious management self-supporting vaccination promises
well in Chota Nagpore.

Superintendent's
inspections.

47. The superintendent visited the first party thrice, the second twice, and the third
once. He travelled over 929 miles during the season. The following is a record of the cases
systematically inspected:—

Prevalence of
small-pox.

Inoculation.

48. Small-pox was again more than usually prevalent in the hot weather of 1874.
284 deaths were reported from Lohardugga, 146 from Hazareebagh, 173 from Manbhoom,
and 29 from Singbhoom, total 632, against 667 in 1873. Vaccinators were employed
for the purpose of stamping out outbreaks when they were reported. Inoculation is still
sometimes stealthily practised. A man was caught near Ranchee and convicted; another man
at Chuttra, and six more in Palamow sub-division. These seizures were effected when small-
pox was prevalent, and as they are probably merely an indication of what goes on, it is not
surprising that small-pox outbreaks are still common in this division.

General conclu-
sion.

49. Although the work of the past season fell short both in amount and success of
that of the previous year, Surgeon Wood's report furnishes good evidence of his continued
interest in his work. Judiciously and energetically managed, the future of vaccination in the
Chota Nagpore Circle is very promising.

SONTHAL
PERGUNNAHS.
Superintendence.

50. Sonthal Pergunnahs.—Vaccination in this circle, whose area is co-extensive with
the district of the same name, is under the general superintendence of the civil surgeon
assisted by a deputy superintendent—an assistant surgeon. Surgeon Zalnoor Ali
Ahmed, M.B., occupied the former position during the past season, and 3rd grade Assistant
Surgeon Sonatun Bysack, relieving 3rd grade Assistant Surgeon Kali Krishna Ghose on
the 27th of August, the latter.
